Vanderheyden Hall Nurse (LPN) - Residential Treatment - Youth Program - Full time / Part time in Wynantskill, New York

Position Summary:
As a member of Vanderheyden's clinical team this nurse will administer medications to the individuals served at Vanderheyden's OCFS Programs in accordance with the New York State Nurse Practice ACT. New York State Office of Children and Family Services, and any/all applicable policies and procedures of Vanderheyden. Licensed Practical Nurses will provide these services under the supervision of a Registered Professional Nurse (RN). Evenings and weekends.
Primary Duties & Essential Functions:
Individual NPI# to be maintained
Comply with all 29I Licensure Requirements
Create and maintain Medication Administration Records (MARS)for each individual enrolled in Vanderheyden's OCFS Programs.
Register the individual with the contracting managed pharmacy services.
The nurse will collaborate with the RN, Direct Care Supervisor and the Case Coordinator to develop ways to teach and monitor individuals and Direct Care Staff regarding performing medication education and personal care.
Ensure and maintain and adequate supply of medication for individuals use in residence and on home visits.
Review the Medication Administration Reports to as certain that they accurately reflect the medication prescription(s)/physician's order(s) and that the documentation of the administration of medication is completed correctly.
Review and prepare individuals' Medication Administration Records (MARS) monthly and send to the living unit to be placed in the Medication Book.
Maintain a complete and accurate medication monograph for contracting pharmacy to be kept in the medication book with the individuals MARS.
Administer medications to individuals at the appropriate times as ordered by their providers.
Provide on-going nursing information to supervising RNs/Nurse -On-Call before leaving shift as dictated by individuals needs
Upon discharge, ensure all medications will be gathered to be sent with resident.
Develop and /or participate in ongoing staff development for employee e as directed.
Ensure knowledge of all medications administered and their intended, side, and adverse effects.
In case of an emergency, if needed, you may be required to assist with the management of an individual’s acute need(s).
Accurately maintain individual's Medication Administration Records.
Timely, accurate, and complete documentation of all medications and medical treatments provided to all individuals
Maintain contact with other Health Services staff regarding individuals' medication and treatment concerns
Maintain regular contact with Direct Care Workers to discuss individual’s medication concerns.
Maintain confidentially in accordance with agency policy and HIPPA regulations.
Participate in required agency orientation and education development
Required Education, Knowledge and Skills:
Promotes and supports cultural diversity and competence
Will be a graduate of an accredited nursing school and /or college degree program.
Will have a valid New York State nursing license that has not been suspended or with any limitations.
Will have documentation of and favorable references that may be checked, as appropriate.
Will have valid, unrestricted New York State driver’s license
Abilities and Working Conditions:
Must be able to lift 25 pounds.
Must be able to stand for moderate periods of time.
Must have a valid driver's license with a clean MVR.
Willingness to respond to the needs of a culturally diverse population.
Pay: $27 an hour
