JOB DESCRIPTION:
A Licensed Practical Nurse (LPN) provides direct care for long-term and sub-acute residents and supervises the duties performed by the Certified Nurse Assistant to ensure the highest degree of quality care. An LPN works as a team member under the direction of the RN, Nurse Manager, Nursing Supervisor, and Director of Nursing in a nursing unit.
RESPONSIBILITIES:
Ensures the care and safety of all residents in the nursing unit
Ensures that individual care plans are followed
Administers and documents medications, treatments, and other nursing services
Makes a count of controlled substances on the assigned nursing unit
Documents current condition of assigned resident in EMR
Receives and gives an accurate report of the resident condition at shift change
Communicates to the nursing team and medical provider as indicated and required
Assists in the orientation of new staff
Maintains competency in all tasks required by CMS
REQUIREMENTS:
Current and valid license as an LPN in the state of New York required
Degree from an accredited school or nursing/nursing program is required
Current CPR certification required
Education or experience in long-term care or geriatric nursing is preferred
Demonstrate effective communication skills, problem-solving, and conflict resolution
Must be flexible, punctual, professional, and reliable
Strong customer service skills and compassion
Basic computer knowledge, PCC e-MAR experience preferred
Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
