Community Mainstreaming Associates DIRECT SUPPORT PROFESSIONAL (DSP) NASSAU in Westbury, New York
Salary Range: $18.00 To $18.00 Hourly
What Your Role Will Be:
Support individuals to reach their personal goals, from daily tasks to personal achievements Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals. Support and promote the individuals health and safety Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines. Assist with personal care, social interactions, meal prep, grocery shopping, and light housekeeping Minimum Qualifications:
High School Diploma or GED required
Valid NYS Driver License with a minimum 1-year driving experience required
Experience as a Caregiver, Direct Care Worker, Nursing Assistant, Home Care Aide or Companion a plus
Experience with people with intellectual and developmental disabilities preferred
Must be able to operate Motor Vehicle
Evening and weekend shifts preferred
Must be able to lift up-to 50 lbs.
Work Locations: Residential houses located in Nassau County (Glen Cove, Kings Point, Massapequa, and Port Washington).
Work Hours/Schedule: Full Time, Part Time & Per Diem. Day & Evening shifts available.
Glen Cove: Mon, 8 am to 2 pm, Tues - Thur, OFF, Fri, 6 am to 2 pm, and Sat - Sun, 8 am to 2 pm (26-hour work week)
Kings Point: Mon - Tues, 8 am to 4 pm, Wed - Thur, OFF, and Fri - Sun, 8 am to 4 pm (40-hour work week) Mon - Tues, OFF, Wed - Fri, 2 pm to 10 pm, and Sat - Sun, 8 am to 4 pm (40-hour work week) Mon - Tues - 2 pm to 10 pm, Wed - Fri, OFF, and Sat - Sun, 2 pm to 10 pm (32-hour work week) Mon - Thur, OFF, Fri, 6 pm to 10 pm, and Sat - Sun, 8 am to 2 pm (16-hour work week)
Massapequa: Sun - Mon, OFF, Tues - Thur, 3 pm to 11 pm, and Fri - Sat, 2 pm to 10 pm (40-hour work week) Mon - Fri, OFF and Sat - Sun, 8 am to 2 pm (12-hour work week) Mon - Fri, OFF and Sat - Sun, 8 am to 2 pm (12-hour work week)
Port Washington: (Medical Counselor/Direct Support Professional position) *Schedule changes bi-weekly based on the needs of the individuals Tues - Sat, on schedule, and Sun - Mon, OFF (40-hour work week) (Tues - Sat, 8 am to 4 pm, 9 am to 5 pm, 10 am to 6 pm, or 11 am to 7 pm)
Westbury: Sun - Mon, OFF, Tues - Wed, 2 pm to 10 pm, Thur, 3 pm to 11 pm, and Fri - Sat, 2 pm to 10 pm (32-hour work week) Pay: $18/hour
