Job Details:

The Registered Nurse is responsible to accept, and complete Home Care referrals received. This position assists in the development of appropriate Home Care plans with the referring institution, physician, patient/family/caregiver and Home Care staff. Additionally, they work with established referral sources and develop new sources for home care referrals as well as increase the awareness of the scope of services that home care can provide for physicians and other health care personnel.

RESPONSIBILITIES:

  • Screens all referrals for appropriateness of Home Care services.

  • Collaborates with Home Health team to identify potential patients for Home Care services.

  • Reviews hospital chart and obtains appropriate information.

  • Collaborates with Case Managers from area hospitals in the coordination of Home Care services.

  • Demonstrates a professional attitude and ethical conduct in compliance with departmental standards.

  • Attends a minimum of six in-service programs annually.

  • Involves the patient and caregiver actively in goal setting.

  • Collaborates with staff regarding patient plan of care and progress.

  • Demonstrates proper teaching techniques in the development and implementation of individualized age specific teaching plan and seeks assistance as needed.

  • Teaches and supervise ancillary staff to gain knowledge and skills to appropriately carry out assigned tasks.

  • Serves as a mentor to staff.

  • Confers with physicians and other members of the health care team to formulate a plan of care including hospital case managers/discharge planners for tracking of hospitalized patients.

  • Provides information and referral services as needed to Physicians and caregivers calling from the community and follows-up on appropriate Home Care referrals.

  • Educates potential referral sources regarding services.

  • Explains services to referral source, patients and caregiver in a concise manner.

  • Seeks opportunities to enhance home care knowledge.

  • Incorporates QI recommendations into clinical practice.

  • Assists in the evaluation and revision of departmental standards of care and/or practice.

  • Demonstrates problem solving in the decision-making process.

  • Serves as liaison while at facility between hospital and the Home Care Agency.

  • Establishes good working relationships with all members of the health care team.

  • Attends monthly staff meetings.

  • Verifies and confirms health insurance coverage for Home Care services and documents coverage on the intake sheet.

  • Demonstrates competency in the interpretation of a patient’s self-report or behavior; interprets information with an understanding of the cognitive, physical, emotional and chronological maturation process.

  • Initiates the Home Care referral process in a knowledgeable, skillful, consistent manner with home care staff and hospital case managers.

  • Arranges for lab and equipment in a timely manner.

  • Demonstrates proficiency and competency in knowledge of Home Care regulatory requirements.

  • Demonstrates competency to follow-up on requests and concerns expressed by referral sources timely.

  • Demonstrates competency to give detailed report to appropriate personnel considering patient needs through collaboration with the Home Care Team.

  • Visits referral sources as per established standards.

  • Demonstrates proficiency and competency in knowledge of Home Care regulatory requirements.

  • Demonstrates competency to complete timely and accurate activity reports.

  • Demonstrates competency in required computer skills.

  • Demonstrates competency in ability to analyze, interpret and report data.

    QUALIFICATIONS / REQUIREMENTS: One (1) year experience in a Certified Home Health Agency or Discharge Planning/Case Management role.

    EDUCATION: Graduation from an accredited school of Nursing required.

Licenses/Certifications: Current registration/license to practice as a Registered Professional Nurse in New York State and BLS required.
