Position Summary Information
Job Description Summary
Provide support to patients to ensure compliance with care. Coordinate services and function as navigator and advocate in assisting patients to access services. Coordinate follow-up with patients for missed clinical appointments and work with them to overcome barriers to care. Ensure that appointments are scheduled and kept. Provide patient education appropriate to the needs of patients. Work within Family Medicine department.
Minimum Qualifications
High School diploma or equivalent required. Healthcare education, certification, and/or training required. Outreach or customer service experience required. Must have excellent customer service, interpersonal and communication skills. Broad knowledge of health and community resources. Ability to work independently as well as with supervision and as part of a team. Demonstrated sensitivity toward the client population serviced. Bilingual proficiency preferred.

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus.
