Position: Community Support Professional (CSP)
Compensation Range: $15.70-$18.95
Hours: Flexible/Variable – Full time, part time and per diem schedules available.
The Community Support Professional will provide in-home and community-based habilitation services to individuals within the Community Habilitation program. The Community Support Professional must be able to support individuals in exploring activities that allow them to develop life skills and achieve their personal goals. Additional responsibilities include but are not limited to:*Being flexible with scheduling based on individual's needs
*Self-motivated, creative and willing to work autonomously
*Providing transportation when necessary to/from individual's home and community-based activities, appointments, etc.
*Documenting services provided as outlined in individual's service plan
Minimum Qualifications: * High school diploma or GED (in lieu of diploma/equivalent, a passing score as outlined on standardized tests may be accepted)
Valid NYS driver’s license which meets agency guidelines
Basic computer skills required, access to a mobile device necessary and access to a home computer beneficial
Previous experience in ID/DD field preferred
