GENERAL DESCRIPTION :

The social worker is responsible for providing individual and group therapy to patients who attend the Child and Adolescent Partial Hospital Service (CAPHS) and their families. He/she conducts psychosocial and family assessments, and in collaboration with the interdisciplinary treatment team, develops and implements treatment and continuity of care plans for patients assigned to his/her caseload. The social worker provides social work consultation to members of the interdisciplinary team. He/she develops and maintains relationships with community agencies and providers who refer to and receive patients from the Service. In conjunction with other members of the interdisciplinary team, the social worker is responsible for crisis intervention, maintenance of the therapeutic milieu, and assessment and management of patient safety.

SPECIFIC RESPONSIBILITIES :

Administrative (5%)

  1. Ensures that Social Work Division, Department, Program, and Hospital policies and procedures are implemented with regard to patient care, discharge planning, and professional practice.

  2. Participates in program development, implementation, and evaluation as delegated.

  3. Participates in routine committee process of the Social Work Division and the CAPHS and complies with Social Work Division and Department quality assurance and staff development activities.

  4. Attends Social Work Division and Program staff meetings, and may attend program or service area Administrative meetings as delegated.

  5. Participates in activities of the Social Work Division, including, but not limited to timely completion of statistical reports/forms, time sheets and other Hospital or Division forms, Hospital and Division mandatory inservice training programs, and annual health updates.

  6. Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  7. Participates in available cultural competence events and training appropriate to job duties.

Clinical (95%)

  1. Serves as primary therapist to patients assigned to his/her caseload.

  2. Performs psychosocial assessments of patients and family assessments.

  3. Participates in the planning and leadership of therapeutic groups for program patients, and contributes to the management of the therapeutic milieu.

  4. Provides social work treatment and education to patients and families. This may be in the following modalities: individual, family, and/or group therapy.

  5. Coordinates activities related to the comprehensive treatment and continuity of care planning for the patients assigned to his/her caseload with other members of the interdisciplinary treatment team.

  6. Collaborates with appropriate collateral contacts and maintains linkages to community resources.

  7. May participate in the intake/screening process for new referrals as requested.

  8. Consults to other members of the treatment team regarding community resources, entitlement programs, Child and Adult Protective Services, and domestic violence.

  9. Documents social work assessments and interventions in the patient’s chart in compliance with Division, Departmental, Program, and Hospital policy.

  10. Participates in on-call activities of the Social Work Division.

  11. Provides back-up social work services as requested during the absence of co-workers.

Teaching (TBD)

  1. The social worker may provide supervision and training to staff and students as determined by the social work supervisor in consultation with the Clinical Manager for Social Work, and the Program Director.

  2. The Social Worker may function as a field work instructor for graduate or undergraduate Social Work students.

Research (TBD)

The social worker may participate in social work and/or interdisciplinary research as determined by the social work supervisor in consultation with the Clinical Manager for Social Work and the Program Director.

PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION :

Supervision/Consultation

The clinical social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).

Accountability

  1. The social worker is accountable clinically and administratively to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description.

  2. The social worker is accountable to the Director of the CAPHS for providing professional social work services in accordance with the policies and standards established by the Program, the Department of Psychiatry, and Strong Memorial Hospital.

Evaluation

Social Work evaluations are conducted annually by the social worker's supervisor with commentary from the Clinical Director of the Program. Salary and promotional recommendations are made by the social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to the guidelines established by the Social Work Division, Strong Memorial Hospital, and the University of Rochester.

POSITION QUALIFICATIONS :

The social worker in this position must have a Master of Social Work degree from an accredited social work graduate program and obtain within one year of employment, a New York State License in social work.

POSITION DESCRIPTION MAINTENANCE :

This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Clinical Director of the Program, and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Director of the Program. Any position description modification is authorized by the Social Work Division Clinical Manager for Psychiatry.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
