The social worker, in collaboration with the interdisciplinary care team, provides social work clinical services, including transfer management, discharge planning, to individuals and families of all ages who come to the Emergency Department for care.

The social worker also provides consultation to inpatient staff regarding psychosocial issues and community resources if the assigned social worker is unavailable (between 4:30PM and 8AM).

Specific Responsibilities

Clinical: 95%

  1. Manages patient transfers to Highland Hospital as per transfer protocol.

  2. Performs risk screening and evaluation of ED and admitted boarding patients age 80 years and above, as well as complaints suggestive of abuse or neglect, alcohol or substance abuse or domestic violence.

  3. Performs psychosocial assessments of patients identified through risk evaluation or referral.

  4. Collaborates with ED and inpatient physician, nursing and Community Health Nursing staff in developing an interdisciplinary care and/or discharge plan for the patient.

  5. Provides social work interventions to patients and their families including emotional support in crisis situations, information about and referral to community resources, facilitation of patient transfer to other care facilities and other services relevant to the patient's discharge from the ED.

  6. Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence.

  7. Notifies the inpatient social workers of admitted patients when the ED social worker has gathered relevant psychosocial information.

  8. Notifies the Social Work Administrator on call of potential non-acute inpatient admissions and collaborates with the Manager in activities to divert the admission.

  9. Provides consultation to other ED staff about psychosocial issues and resources for individual ED patients.

  10. Provides consultation upon request to other hospital units regarding psychosocial questions and community resources if assigned social work staff is unavailable.

  11. Develops and maintains knowledge of hospital and community resources relevant to the needs of ED and admitted boarding patients and their families.

  12. Notifies SMH Pediatric consultant of all possible cases of abuse or neglect. Collaborates with CPS to ensure safe & appropriate discharges of Pediatric patients with complaints suggestive of abuse or neglect.

  13. Documents all activities in the patient's ED chart, or, in the case of other unit patients receiving direct social work service, in the patient's hospital medical record, in accordance with the Social Work Division policy and procedure.

  14. Provides coverage services that are requested during the absence of co-workers. Including shift modification in order to accommodate ED social work absences including days, evenings, and overnights.

  15. Performs other duties as assigned.

Administrative: 5%

  1. Participates in ED social work staff meetings.

  2. Participates in social work supervision/consultation about his/her work in accordance with plan developed with supervisor.

  3. Participates in required activities of the Social Work Division and Hospital including, but not limited to, timely completion of statistical reports/forms, time sheets and other Hospital or Division forms; mandatory inservice training programs and University Health Service reviews.

  4. Participates in social work tasks and committees as assigned to develop policies, procedures and information required for the social work program in the Emergency Department.

Professional Accountability, Supervision and Evaluation

Supervision/Consultation

The Senior Social Worker and the Manager of Adult Social Work Services supervise the social worker’s professional performance.

Accountability

  1. The social worker is accountable administratively and clinically to the senior social worker and Manager of Adult Social Work Services for the provision of social work services in accordance with Social Work policies, procedures and standards of conduct. The social worker’s job performance must conform to the expectations set forth in this position description.

  2. The social worker is accountable to the Director of the Emergency Department for providing social work services in accordance with policies and procedures established by the Emergency Department.

Evaluation

  1. Social Work evaluations are conducted by the senior social worker and Manager of Adult Social Work Services at the end of the first six months of employment and at least annually thereafter. Salary and promotional recommendations are made by the Associate Director in accordance with guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

Position Qualification

The social worker must have a Master’s Degree in Social Work from an accredited social work graduate program with relevant experience and must obtain New York State licensure within 18 months from date of hire.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
