SMH Department of Ophthalmology and FLAUM Eye Institute FEI Faculty Practice and Center for Perioperative Medicine require a Social Worker to serve as a liaison with community agencies and other relevant providers. It is the responsibility of the Social Worker to provide interventional services to patients who are seen by these clinics/departments. The Social Worker will assist FEI and CPM professionals and support staff in the provision of comprehensive care to patients. The social worker consults with health care providers regarding patients' care needs and/or community resources. Social Work services include psychosocial assessments, assisting with pre-admission and pre-surgical discharge planning, coordination, informational and resource referrals. The Social Worker will service the Flaum patients in Same Day Surgical Unit.

Specific Responsibilities

Clinical

Essential Functions 95%

The Social Worker:

  1. Conducts pre-admission and post-admission screenings, psychosocial, Social, and discharge planning for patients identified by support staff as being at psychosocial risk; facilitates information exchange, and promotes communication to optimize the patient's success after surgery, promoting quality of care and safety.

  2. Performs psychosocial assessments to identify problems and provide services. These services include, but are not limited to. Crisis intervention; supportive counseling; problem-solving; information and referral; assistance with environmental factors affecting a patient's care plan and overall health outcomes.

  3. Coordinates referrals to community agencies and serves as a liaison for services to patients, their families, and caregivers. Collaborates with community groups to develop resources for patients and their support system

  4. Maintains a current understanding of community resources relevant to clinic patients. Providing consultation related to patient problems and community services to other clinic providers.

  5. Participates in the development and implementation of interdisciplinary care plans with other clinic providers.

  6. Assesses and intervenes appropriately for children and adults who are suspected of being victims of abuse, neglect, and/or domestic violence.

  7. Participates in professional development programs. Participates in relevant training and educational opportunities designed to enhance knowledge and skills in intervention and protective work.

  8. Ensures that all social work services are documented in the patient's medical file according to the policies and procedures of the Medicine Department and the Social Work Division.

Non-Essential:

The Social Worker:

  1. When appropriate, makes home visits to patients' homes or community agencies to assist them with the implementation of their care plans.

  2. Provides coverage services as requested during the absence of co-workers (planned and unplanned).

  3. Performs other duties as assigned.

Administrative

Essential Functions 5%

The Social Worker

  1. Attends Social Work Division and social work unit staff meetings.

  2. Completes Social Work Division Daily statistics in eRecord, Time Distribution and other required forms and documentation within the designated deadlines;

  3. Serves as a member of any CPM/FLAUM Clinic committees and administratively, the Social Worker will participate in the CPM quarterly QI processes and is invited to attend educational lectures and seminars presented to the APP and RN staff.;

  4. This Social Worker takes part in interdisciplinary collaboration for program development, research, reporting, coaching, mentoring, and supporting program initiatives.

Non-essential:

The Social Worker:

  1. Serves on committees of the Social Work Division;

  2. Assists other hospital, community, social work, and patient groups in developing and expanding services related to CPM and FLAUM.

  3. A Social Worker attends and participates in departmental, hospital, professional, and relevant community meetings. Also serves as a liaison with community health and social service agencies. The social worker may serve on advisory boards and coalitions in the community

Supervision and Teaching

The social worker:

  1. May participate in training of hospital staff of all disciplines and volunteers regarding the function of the CPM/FEI social worker and/or the psychosocial needs of patients/families; The Social Worker participates in activities of the Social Work Division including but not limited to mandated training, health updates and professional development.

  2. The Social Worker will also be responsible for educating the CPM staff on relevant topicss that impact safe discharge planning including elder abuse/neglect, domestic violence and discharge safety after anesthesia/surgery in partnership with CPM medical team.

  3. May supervise BSW or MSW social work students after 1 full year of employment.

Professional Accountability, Supervision and Evaluation

Supervision/Consultation

The social worker’s professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LMSW). Supervision schedule will be set up by supervisor and social worker.

Accountability

  1. The social worker is accountable administratively and clinically to his/her social work supervisor and through him/her to the Director of Social Work and Patient & Family Services for the provision of social work services and adherence to Social Work and Hospital policies, procedures and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in this job description.

  2. The social worker is accountable to the CPM/FLAUM Center Medical Director for providing social work services in accordance with policies and procedures established by the CPM/FLAUM and URMC Organizations.

  3. The social worker is expected to demonstrate the Strong Commitment-“I Care” values.

Evaluation

Social work evaluations are conducted initially at six months and annually thereafter by the social worker’s supervisor. Salary and promotional recommendations are made by the social worker’s supervisor, in consultation with the Director of Social Work in accordance with guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

Position Qualifications

The social worker who occupies this position must have an MSW from an accredited school of social work and must obtain and maintain New York State licensure (LMSW) within 18 months of employment.

Position Description Maintenance

The position description is reviewed annually by the social workers and their supervisor. Position review provides opportunity for comment from appropriate CPM/FLAUM Center faculty and for updating as needed. The position description is also reviewed and updated at the time that a new social worker is hired for the position. Changes in the position description are recommended by the supervisor. Position description modifications must be authorized by the Director of Social Work and Patient & Family Service.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
