The Team Lead will lead and/or provide oversight to a team of clinicians and support staff in providing direct nursing services to patients. This role may include assisting in orientation of new staff, role modeling, co-visiting to evaluate or demonstrate clinical skills, participating in quality assurance activities, identifying in service needs, designing in-service programs to address identified needs and supervising staff.
ROCHESTER REGIONAL HEALTH HOSPICE HOME CARE
Our Hospice Home Care team provides hospice and palliative care services within the comfort of our patients' homes. Our Nursing and Therapy teams work directly with patients and support their families through the loss of a loved one. We partner with our Hildebrandt Hospice Care Center, our hospitals and health facilities to provide end to end continuity of patient care. We are a responsive, collaborative partner in advocating for, and providing access to effective, innovative, culturally competent and cost-efficient services for the communities we serve.
STATUS: Part Time
LOCATION: Rochester, NY
DEPARTMENT: Palliative & Hospice Home Care
SCHEDULE: Weekends, Day Shift
ATTRIBUTES
- Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
- Valid RN Licensure in NYS and at least 1 year of Home Care experience
- Demonstrate strong leadership/supervisory skills.
- Must demonstrate solid interpersonal, organizational and time management skills.
- Must have ability to travel.
- Patient Care & Service. Ensures that referrals for services are appropriately assigned in a timely manner.
- Planning. Leads the orientation of new staff by conducting orientation classes, providing observation and co-visits, and assisting with case management as appropriate. Helps to drive the ongoing development of selected clinical staff through provision of assistance in case management or other clinical services.
- Communication. Oversees/guides staff in the provision and/or development of services to patients in addition to ensuring appropriate and accurate clinical documentation standards. Serves as resource person to agency staff in areas of clinical expertise and participates in development of special clinical programming as appropriate
- Documentation. Participates in the identification, design and implementation of area in-service programs.
- Compliance. Maintains knowledge of all relevant legislative and regulatory mandates and ensures that all activities are in compliance with these requirements.
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE: $28.37 - $52.88
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
