SUMMARY
The licensed practical nurse performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a licensed health care provider.

LOCATION: Canton-Potsdam Hospital

SCHEDULE: 9.5-hour shifts, 24 hours per week, variable shifts with on call responsibility
ATTRIBUTES
- Required: Graduate of a practical nursing program. Displays knowledge of medications, their administration, and untoward effects.
- Preferred: Graduate of an accredited practical nursing program
- Required: Current, unrestricted New York State practical nursing license
- BLS certification within three months of employment
- Preferred: One year of post-licensure practice as a practical nurse
- Collects patient data to be used by the licensed health care provider in the comprehensive care of the patient. Obtains vital signs, allergies, current medications, medical history, social history, family history, and chief complaint, and documents them appropriately in the electronic medical record.
- Responsible to identify problems within the practice and communicates them to their manager. Assists in maintaining a clean and organized department to ensure a safe patient care environment. Supports all initiatives of the health care team and promotes excellence in customer service. Works with PPM team to resolve clinical issues and identify opportunities for improvement.
- Performs and documents direct patient and family centered care according to nursing standards of practice. Prepares charts for the providers in order to assure that the needed information is present on each chart before the patient is seen in the office.
- Provides routine nursing care and treatment to patients under the direction of the licensed health care provider, collects specimens, performs medication reconciliation and administration, and assists providers with simple procedures. Supports and adheres to administrative and nursing service policies and procedures.
- Efficiently uses medical and clerical supplies in order to help effectively control practice expenses. Appropriately documents all treatments performed during patient encounters and adds the appropriate CPT code as required.
- Collaborates professionally with all members of the health care model in the provision of patient care. Works with PPM team to comply with all established standards required under the accreditation standards of DNV. Is knowledgeable of state and federal health care standards, as appropriate. Adheres to CPH and PPM policies, procedures, guidelines, and protocols.
Unit Specific Details:
- Under the direction of a Physician, Midlevel Provider or RN, provides direct and individualized nursing care to assigned patients following established nursing policies and procedures and applied scientific principles. Incorporates age-specific, cultural, spiritual and end of life needs of the patient and significant other (s.)
- Assists in patient assessment care process, by observing, collecting and reporting objective and subjective data regarding patient's medical/surgical history, allergies, medications, physical and psychological status. Reports patient condition changes and/or signs and symptoms of deviations in a timely manner, maintaining confidentiality at all times.
- Checks supplies, instrumentation, and equipment needed for medical procedures and ensures proper operation. Opens sterile supplies and sets up the table for surgical procedures.
- Provides a sterile field for surgical procedures. Assists the Physician with draping the patient to isolate the operative site. Assists in the maintenance of the sterile field by applying principles of aseptic techniques and informing of any infractions.
- Passes instruments and supplies to the Physician.
- Prepares, administers and records prescribed medications in accordance with established hospital, regulatory policy and guidelines.
- Demonstrates effective documentation and communication skills. a. Supports and collaborates with the multidisciplinary team approach. b. Supports the Shared Governance Concept.
- Encourages an environment that optimizes patient and environmental safety and in accordance with applicable laws, regulations and compliance effort. a. Proactively identifying potential and actual patient safety risks, identifying underlying causes and making the necessary improvements through a systems-based approach. b. Operates and maintains equipment utilizing proper procedures, established policies and reporting of malfunctioning equipment. c. Communicates issues of concern and safety in a timely, appropriate manner.
- Fulfills professional obligations a. Maintains all education, licensure, certification and regulatory requirements. b. Adheres to the CPH Standards of Behavior. c. Fosters an environment conducive with the standards of Nursing Practice. d. Identifies and promotes opportunity for professional growth. e. Completes all hospital wide and unit based competencies Age Specific Care: Provides age and culturally appropriate care and carries out appropriate interventions and plans of care including education within the parameters of his/her position responsibilities and licensure.
- Minimum Education/Training: Graduate from an approved licensed practical school of nursing. Licensure/Certification: Current LPN license to practice in the State of New York. Annual BLS certification required. Experience/Skills/Abilities: Previous experience assisting in a surgical environment preferred. Successful completion of annual clinical, age and job specific competencies and skill verification tools required. Excellent verbal, written, organizational and critical decision-making skills required. Good interpersonal skills and ability to deal with all levels within the organization and community under varied conditions
PAY RANGE: $27.59 - $38.70
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
