Overview

The Targeted Outreach Worker, under the supervision of the Program Coordinator, is responsible for planning and conducting outreach activities outlined in the annual work plan of the Cancer Services Program (CSP) to increase the number of uninsured Clinton, Essex, Franklin County residents, in the eligible population, screened for breast, cervical, and colorectal cancer. Responsibilities will also include direct engagement in the community. Recruits previous clients and provides other program support as needed. The Targeted Outreach Worker's responsibilities and job focuses are dictated by the state and may change at any time depending on what is required by the Cancer Services Program Grant.

This is a grant funded position.

Responsibilities

Develop and maintain relationships with community-based partners to refer eligible, priority population to CSP.
Organize and conduct public cancer screening awareness and education events.
Under supervisor of the Program Coordinator, prepare advertisements, medial releases, flyers, posters, etc. appropriate to target CSP priority.
Conduct outreach activities that aim to achieve screening objectives according to the CSP work plan.
Maintain the existing system to recruit clients for rescreening through CSP.
Motivate and encourage members of the priority population to complete all screening exams for which they are eligible.
Attend networking meetings and community events appropriate for promotion of CSP services.
Maintain a comprehensive log of activities and the outcome of each to review with the CSP Program Coordinator and NYSDOH Regional Manager.
Attend all program trainings/meetings required by NYSDOH which may include overnight travel.
Attend/participate in bi-weekly or monthly staff meetings and calls with the Program Coordinator and NYSDOH, as required.
Complete all required reporting on the Catalyst system and requests from NYSDOH, related to outreach activities.
Screen clients for eligibility.
Schedule client appointments.
Maintain database for enrolled clients.
Follow up with clients who do not complete testing and address barriers.
Submit client information and results in New York State Catalyst online system.
Maintain contact and good working relationship with participating offices.
Organize and attend screening all screening events (minimum of 3 per year).
Provides statistical client and monetary reports as needed.
Answer phones and direct calls.
Adopt and complete any new program or process required by New York State Department of Health grant deliverables.
Other duties as assigned.

Qualifications

High school graduate or GED required.
2 or 4 year degree in health education preferred.
Excellent communication and organization skills required.
Microsoft Excel and Word Course Certificates required, or evidenced by passing a CVPH test requirement.
Ability to type 40 Net/Adjusted WPM preferred.
Working knowledge of Clinton, Essex and Franklin County population/demographics preferred.
Thoroughness, attention to detail, and follow through until problems are resolved required.
Customer service skills required.
Successful completion of a CVPH medical terminology exam required.
Working knowledge of breast, cervical, and colorectal cancer preferred.
Experience with community resources preferred.
Public speaking experience required.
