Description
Reports to:Medical Director / Practice Manager
Rate of Pay:$53,800 - $64,100 annually for full-time status; part-time or per diem shall be prorated based on scheduled hours
Work Schedule:Flexible within Monday - Friday daytime office hours; no evenings, weekends, or major holidays; no on-call; full-time, part-time, or per diem schedules available
Position Summary:Provides counseling services to health center patients in compliance with standard practices.
Primary Responsibilities:
Perform psycho/social assessment.
Develop a therapeutic counseling regimen to meet the needs of the individual based upon assessment and input from caregivers/family.
Coordinate treatment plan with the psychiatrist, referring individuals as necessary for psychiatric consult and prescription.
Provide ongoing counseling as indicated and follow up if psychiatric consultation is made.
Maintain patient records per agency policies and procedures.
Provide ongoing supervision of MSWs as necessary.
Perform other duties as requested by the supervisor.
Requirements
Master’s Degree in Social Work
Current Registered NYS LCSW license
Experience with electronic medical records is preferred.
Strong organizational and planning skills.
Ability to maintain good working relationships with others.
Exhibits a high level of professionalism and confidentiality in dealing with issues.
Ability to lift and carry up to 25 lbs.
Ability to sit for long periods.
Ability to tolerate exposure to computer monitor/screen.
Ability to tolerate the extended use of a computer keyboard.
