We are searching for a Licensed Clinical Social Worker (LCSW), who is devoted to providing top notch, evidence-based diagnosis, and treatment to patients. In this role, you will provide psychosocial evaluation, counseling, and resource referral services to patients with a cancer diagnosis. This position is entry level for recently licensed social workers who have earned their Master of Social Work (MSW), but also offers a unique opportunity for career growth by developing invaluable competencies, honing your clinical skills, and overseeing therapeutic programming. Team Impact As an essential part of the interdisciplinary team, you will have the opportunity to provide direct clinical services such as individual counseling, family counseling and psychological evaluation to our patients who have an cancer diagnosis. You will work with other members of the interdisciplinary team to ensure that the psychosocial needs/concerns of patients are incorporated into their comprehensive plan of care. You will also work with your colleagues to assist in the development of various resources to meet all aspects patient-specific needs, and participate in orientation, education, and in-service activities for patients/family members/staff that promote understanding of the psychosocial, patient specific/age related impact of cancer. How You Will Fulfill Your Potential Responsibilities * * Provide psychosocial evaluation, counseling, and resource referral services to patients * Educate families and caregivers on their involvement in the diagnostic/treatment process * Strive to obtain a holistic understanding of patient through the history that is obtained * Document findings from diagnostic assessments and caregiver reports (if applicable) * Collaborate with interdisciplinary team as well as, internal and external stakeholders of Glens Falls Hospital, in developing a plan for the patient that will: * o Involve caregivers where available * o Involve institutional and or, public assistance where necessary * o Incorporate, as much as possible, the history on each patient in an effort to optimize their care with compassion and dignity * o Facilitate resources for both patients and their families * Create steps to further interdisciplinary care by documenting the visit summary and care plans, and identifying appropriate resources of clinical care and social support, respectively * Coordinate interdisciplinary treatment efforts with treatment team members, other services, other providers to maximize quality & effectiveness of care * Provide clinical training and supervision to other staff and/or students as needed * Contribute professional expertise to foster a greater understanding of biopsychosocial care approach for patients with a cancer diagnosis * Facilitate an understanding of the systems in-place which are designed to provide support for both the patients and their families * Represent the assigned clinical program at planning and coordination meetings *Qualifications/Experience * * Education/Accredited Programs * · Master’s degree in Social Work required *Experience/Abilities · Experience providing direct in-patient clinical service strongly preferred · OPWDD experience or training highly desired * Licenses/Certificates/Registrations:* · Must be a Licensed Social Worker in New York State or eligible for state licensure/certification as clinical social worker · Oncology Social Worker certification within one year of hire · Eligible for R number to provide and bill for counseling services * Skills/Abilities* · Strong knowledge of best practice when it comes to providing assessment, diagnosis, treatment, with a sensitivity to racial and cultural diversity · Capacity to form and maintain therapeutic relationships with patients with chronic/terminal illness and their families · Ability to behave in a professional, respectful, and positive manner with patient, staff, and associated providers · Ability to hold a calm demeanor when coping with difficulties/conflict · Ability to ensure client confidentiality, while maintaining clear personal/professional boundaries · Ability to be dependable, manage time effectively, and function autonomously · Ability to manage a large clinical caseload and set priorities under pressure · Ability to form and maintain effective working relationships with interdisciplinary team members · Ability to assess needs and develop resource information and programs · Ability to accept and use supervision/constructive feedback · Ability to develop educational materials and to teach in-service sessions · Ability to demonstrate and maintain commitment to social work values and code of ethics · Skills in advanced assessment and treatment including individual, couple and family therapy · Strong written and verbal communication skills · Effective communication and conflict resolution skills · Refined collaborative skills Communities We Serve Located in the foothills of the beautiful Adirondack mountains, Glens Falls is conveniently located a short drive away from the capital region and Lake George. Salary Range * The expected base rate for this Glens Falls, New York, United States-based position is $27.02 to $38.91 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
