We are looking for a compassionate LPN with the "Hospice Heart" to join our care team who are committed to providing both medical and emotional support to patients and families. Job Details Our Community Hospice LPN’s: Work under the supervision and assignment of the registered nurse to provide top-notch care to hospice patients and their families within a home setting. Communicate status of patient to team Help patient manage pain and end of life symptoms Experience in home care, skilled nursing facilities and/or oncology preferred. Minimum Qualifications: Minimum of one to two years prior bedside nursing experience required Current and valid NY State license to practice as a Licensed Practical Nurse Must have a current, valid New York State driver's license and own transportation Experience in home care, skilled nursing facilities Salary Range USD $32.00 - USD $40.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
