Job Description:
VIA Visually Impaired Advancement is seeking a part time Community Resource Navigator to work 18 hours per week through the duration of this grant supported role.
The C ommunity Resource Navigator will coordinate the Human Services needs of the community members accessing services through Community Action of Orleans & Genesee and will build supportive relationships with families to identify barriers to self-sufficiency . They will assist customers in achieving independence from public assistance and teach them how to effectively use the various resources available. .
Essential Functions:
Work is performed at the Community Action worksites in Orleans and Genesee Counties (can be any office as needed)
Work with community members to identify needs, set goals, implement solutions, and monitor and report outcomes. Refers to appropriate services to achieve whole person care.
Will interview and assist customers through direct agency services and advocate for their needs; emergency services, referrals and/or resolution of housing issues.
Maintain applicable paperwork , monthly reports, c ustomer files and case notes.
Promote and strengthen relationships with other Human Services Agencies
Work collaboratively with agency staff and community partners to establish clear communication to effectively respond to the needs of community members
Complete follow ups and log interactions in accordance with data procedure s
Uphold quality assurance goals
Attend staff meetings, team meetings, and trainings to improve professional skills
Communicate regularly with teammates & supervisor
Stay informed on broad social issues and changes that impact human services in the region
Work on additional projects as assigned
Assist in resource database work as needed
Competencies:
Proficient in the use of Microsoft Office Suite
Demonstrate excellent internal and external customer service
Effectively work with individuals of diverse backgrounds
Comfortable communicating with individuals in crisis or experiencing high levels of stress
Counsel individuals with an ability to balance agency policy while displaying sensitivity to clients/colleagues

Qualifications:
High school diploma/GED required
Bachelor's degree in Human Services or the equivalent experience in Human Services in lieu of degree preferred
Minimum one year experience in Human Services required
Regularly sit for a minimum of 2 hours at a time
Regularly stand for a minimum of 2 hours at a time
Regularly perform repetitive motion of the wrists, hands and/or fingers
