Job Summary:

The Patient Care Liaison is responsible for bringing in clients to the Adult Day Program, by contacting, community resources and prospective clients, in person and via the telephone. The Patient Care Liaison evaluates applicants for admission to the program, and facilitates the admissions process by serving as the liaison between The New Jewish Home (TNJH), applicants, their insurance carriers, and community resources. Assists applicants in navigating insurance to obtain authorization to attend the program.

The Patient Liaison will

· screen referrals for appropriateness,

· coordinate and conduct tours of the facility,

· handle all telephone contacts for referrals

· schedule intakes for 1st and 2nd Days of evaluation ensuring that applicants are screened by clinical team,

· ensure all Admission documents are completed timely

· send out for initial MD order; schedule admissions for follow-up with medical Director is orders are not received within 5 days

· demonstrate understanding of clients insurance and assist in obtaining assessment authorizations.

· ensure that Marketing activities are logged accurately and updated CRM to reflect all activities provided to prospective clients

Requirements:

  • Experience as a Patient Care Liaison in a Nursing Home

  • Network with local hospitals & physicians’ offices (and Assisted Living Organizations)

  • Must possess a thorough knowledge of principles of effective communication, mass media, advertising, policies, education, community relations, demonstration, organization structure, social and activity services, government relations, etc., as they relate to nursing facility operations.

  • Must possess the ability to plan, organize and effectively present ideals and concepts to community groups/agencies.

  • Must possess the ability to communicate effectively, orally and in writing.
