We are currently hiring for a Self-Directed Support Professional, to work within our Catholic Charities Disabilities Services agency. As a Self-Directed Support Professional, you would be responsible for:

  • Providing direct services to people with intellectual or developmental disabilities in accordance with an individualized life plan designed and developed by the individual receiving services

  • Services are provided individually within the community and/or at home, rather than within the structure of any kind of group program. The individual receiving services directs those activities and the SDSP is to guide, facilitate and support the individual in attaining those goals and objectives according to the plan of services

  • In some cases, the SDSP will provide services toward supported employment

  • The SDSP is expected to assist the person with developing meaningful relationships

Self-Directed Support Professional Minimum Salary: $15/hour for training, service rates for Com Hab and Respite are dependent on individual’s Self-Direction Budget

Self-Directed Support Professional Maximum Salary: $15/hour for training, service rates for Com Hab and Respite are dependent on individual’s Self-Direction Budget

Self-Directed Support Professional Schedule: Flexible schedule; full-time and part-time shifts available

Qualifications:

  • Valid NYS Driver’s License and personal transportation (mileage reimbursed)

  • High School Diploma or GED

  • Smart Phone and/or Tablet is required for electronic record keeping
