Description:

• Review medical records.

• Speaking to patients on the phone in hopes of deterring them from going to the ED in an effort to assist them with finding other solutions with the issues they are experiencing.

• MUST be comfortable on the phone speaking to patients.

• Case tracker and EPIC care connect systems will be used to document all discussions, so being able to take accurate, clear, and organized notes is a requirement of this position.

• Have an understanding of sites of service (urgent care, emergency department, etc.)

• Familiarity of health records is a plus.

• 20 hour work week with potential for extension.
